Why do I have to go to a "Designated Provider"?
By statute, the employer can designate medical providers for workers'
compensation. Reasons to designate providers: - to provide employees
with appropriate medical care for on-the-job injuries from
occupational / industrial medicine doctors - to give the employer a
working relationship with the medical provider to help employees get
back to work on modified duty.
One of the most common question an injured worker asks is can I CHOOSE
to see my own doctor or licensed healthcare processional?
By South Carolina law you must first go to your doctor chosen by your
employer or your employers insurance representative . Depending on the
nature of your injury it may be important that you first get a medical
evaluation by a qualified physician.
You have the right to
request your own therapy and your own licensed healthcare therapist
this includes medical massage therapy.
If you want to include medical massage therapy as part of your
treatment plan, authorization must be obtained from your employer and/or
employer's health insurance adjuster.
So the big question is what is the process to get
workers' compensation to cover your work related injury at the Medical

Steps to receive
authorization for medical massage treatment:

Contact the medical doctor
who was designated to you by your employer and/or employer's insurance
carrier and present them with a
Request For Medical Massage Therapy Form A. This form requests that
your medical doctor provide you with a letter of medical necessity so
that your medical massage therapy will be included as a part of your
treatment plan.

Contact your human
resources department of your company and present them with a Request
For Medical Massage Therapy Form B. This form requests that your
employer authorize medical massage therapy as a part of your treatment
plan.

Contact the claims
adjuster of your insurance carrier and present them with the
Request For Medical Massage Therapy Form C. This form requests
that your workers' compensation insurance carrier authorize medical
massage therapy as a part of your treatment plan.
